BLEVINS MIDDLE SCHOOL ATHLETIC REGISTRATION

This registration session will allow you to complete contact information and sign up for participation. Please read through each page carefully to ensure that all information is provided accurately.  If you are a Home-Based, Charter or Private School Student, you must register in person with the PSD school where you compete athletically.  

Important-After completing online registration and paying the athletic fee through SchoolPay, bring your current physical to the athletic office to be cleared for participation. Current physicals must be turned in before athletes will be cleared to participate.  A physical is good for 365 days and valid for all sports throughout the school year. Physicals cannot be uploaded or completed online.

The Health and Wellness Center at Centennial High School offers sports physicals/well-child exams June - August. The Center can bill insurance and no child is ever turned away for inability to pay. For more information visit  www.hwcenter.org or call their office at 970-488-4950.

If you choose to complete the online registration, you will receive an email confirmation.  Once online registration is completed you will have the option to pay the fee either with a credit card or cash/check.  Cash and check payments must be delivered to the school athletic office or bookkeeper. 

PSD Non-Charter Student Fee:   $140 per sport

Home-Based, Charter and Private School Student Fee:  $210 per sport

• Unified Basketball Fee (6th-8th grade): 
  • $70 for PSD Non-Charter Students
  • $105 for Home-based, charter and private school students
