Follow these steps for Fall Middle School Sports

You will need the following information to begin this process-
-PSD Student ID#
-Date of last physical exam for student athlete
-Emergency contact information
-Medicine, tetanus booster and allergy information

1. SPORTS ENGINE REGISTRATION--This registration session will allow you to complete contact information and sign up for participation in the spring season of the 23-24 School Year. Please read through each page carefully to ensure that all information is provided accurately. When you complete the online registration, save your email confirmation.

2. PAY THE ATHLETIC FEE FOR THE CURRENT SPORT--Use your current School Pay Account and save the receipt. (Cash and check payments can be made with the TMHS bookkeeper prior to completion of the next step)

PSD Current 6th to 8th Grade Student Fee:  $140 per sport

Home-Based, Charter and Private School Student Fee: $210 per sport

Unified Basketball Fee (6th-8th grade): 
$70 for PSD Current Students
$105 for Home-based, charter and private school students

3. SUBMIT YOUR CURRENT PHYSICAL--starting August 7th for all Middle School Fall Sports, bring the hard copy of your current physical and proof of payment to the TMHS Athletic Office to be cleared for participation. A physical is good for 365 days and valid for all sports throughout the school year. If we have it on file already, just bring your proof of payment. New Physicals cannot be uploaded or emailed.

The Health and Wellness Center at Centennial High School offers sports physicals/well-child exams. The Center can bill insurance and no child is ever turned away for inability to pay. For more information visit  www.hwcenter.org or call their office at 970-488-4950.
